We're working with this carrier to allow all forms through SureLC, and 99% of them have been approved.  However, Assurity has asked us to not include any ANNUALIZATION forms.

 We hope to have this changed in time, but at this point we are not be able to upload this particular form request.  Assurity requires wet signatures from every upline on this form to ensure their authorization for the producer's  annualization request.
