Why are TRANSFER forms included in CONTRACT Requests?

Allianz  requires the Transfer Request form be included with all contract requests where the producer has had a past relationship with Allianz, regardless of the current status of that relationship today.

At Allianz's request.......In an effort to help alleviate NIGO (Not In Good Order) results, we've included  the Transfer Form with ALL Contract and Transfer requests. 

Please note:  The Transfer form requests the Agent Number transferring from (If Applicable).  If this is unknown, the form will automatically populate the producer's SSN#.  This was created by design.......  If the past agent number is unknown, or simply isn't provided, Allianz will use the producer's SSN# to locate any past relationships.
