Add CE Transcript title option

I request agents to send me a CE Transcript that will give dates of all CE courses if they do have the certifcates for LTC etc.

Also when downloading PDF's it would be good if the pages would stay together. As the CE Transcript is usually 2 to 3 pages.

Another suggestion would be that if we wanted to name a download we could. Example would be Letter of Explanation, Bankruptcy discharge etc.

We plan on including an increased functionality for the different document types in the near future.
